Returns, Refunds, and Service Policy

At Sublimeu Health & Wellness, our services include medical, psychiatric, functional medicine, wellness, aesthetic, health optimization, laboratory-related, prescription-related, and consultative care. All services are provided based on individualized clinical assessment, patient goals, medical history, laboratory findings, safety considerations, and provider judgment.
Because our services involve professional time, clinical evaluation, documentation, medical decision-making, appointment time reserved specifically for the patient, prescriptions, treatment planning, care coordination, customized products, and/or medical review, all sales are final and non-refundable, except where otherwise required by applicable law.
By booking an appointment, purchasing a service, paying a deposit, submitting payment, or purchasing a product, the patient acknowledges that they have reviewed and accepted this Returns, Refunds, and Service Policy.
No Guarantee of Results
Medical, psychiatric, wellness, functional medicine, aesthetic, weight management, hormone-related, and health optimization outcomes vary from person to person. Sublimeu Health & Wellness does not guarantee specific results, symptom improvement, weight loss, aesthetic outcome, medication response, treatment success, diagnosis, prescription approval, lab outcome, or timeline of improvement.
Results may depend on many factors, including but not limited to age, medical history, genetics, metabolism, hormones, nutrition, activity level, medication adherence, lifestyle, sleep, stress, underlying medical conditions, laboratory findings, follow-up compliance, and patient adherence to the recommended treatment plan.
Final Sale / No Refund Policy
All consultation fees, follow-up fees, medication management visits, psychiatric visits, functional medicine visits, health optimization visits, lab review visits, aesthetic services, wellness services, treatment planning fees, administrative fees, portal/message fees, refill request fees, form fees, letter fees, deposits, prepaid services, packages, memberships, promotions, and products are final sale and non-refundable.
Fees are charged for professional time, clinical review, assessment, documentation, decision-making, medical judgment, treatment planning, and appointment time reserved, regardless of whether a prescription, medication, treatment, lab order, referral, clearance, diagnosis, or specific outcome is provided.
Massachusetts Consumer Disclosure
This policy is intended to be clearly available to patients before booking, purchase, or payment. Patients are encouraged to review this policy before completing any transaction.
Nothing in this policy is intended to limit any rights that may apply under Massachusetts law or other applicable law, including rights related to defective merchandise where such rights cannot legally be waived.
Consultations and Appointments
Consultation fees and appointment fees are non-refundable once scheduled, reserved, or completed. The fee covers the provider’s time, clinical review, assessment, documentation, treatment planning, and medical decision-making.
Completion of a consultation does not guarantee that a medication, prescription, refill, treatment, injection, procedure, medical clearance, lab order, referral, or specific recommendation will be provided. All treatment decisions are based on clinical appropriateness, safety, patient history, lab results, medication risks, and provider judgment.
If a patient is determined not to be a candidate for a requested service, medication, or treatment, the consultation fee remains non-refundable because clinical time and medical decision-making were still provided.
Prescription and Medication Approval
Submitting forms, requesting a medication, requesting a refill, or completing a visit does not guarantee that a prescription will be issued. Prescriptions are provided only when clinically appropriate and safe.
Sublimeu Health & Wellness reserves the right to deny, pause, discontinue, or modify any prescription or treatment plan based on medical history, lab results, side effects, contraindications, safety concerns, lack of appropriate follow-up, noncompliance, or provider judgment.
Requests for controlled substances, psychiatric medications, weight management medications, hormone therapy, peptides, compounded medications, or any other prescription medication require appropriate clinical evaluation and do not guarantee approval.
Laboratory Fees
Laboratory fees are non-refundable once labs are ordered, collected, processed, shipped, resulted, reviewed, or submitted to the laboratory. Additional labs may be recommended based on clinical findings, patient history, medication monitoring, safety requirements, or provider judgment.
Lab fees are separate from consultation fees unless specifically stated in writing. Abnormal, delayed, incomplete, canceled, or unexpected lab results do not guarantee a refund.
Medications, Prescriptions, Supplements, and Medical Products
Due to health, safety, hygiene, storage, and regulatory reasons, medications, compounded prescriptions, supplements, injections, hormones, peptides, skincare products, wellness products, and any customized or personalized medical products cannot be returned, exchanged, or refunded once purchased, prescribed, prepared, dispensed, shipped, opened, picked up, or released to the patient.
This includes products that are not suitable for return due to health protection, hygiene concerns, expiration dates, temperature control, storage requirements, contamination risk, or individualized medical use.
If there is a concern about a damaged, defective, incorrect, or compromised product, the patient must contact Sublimeu Health & Wellness promptly. Any available remedy will be reviewed on a case-by-case basis and may depend on the type of product, pharmacy policy, manufacturer policy, laboratory policy, shipping documentation, and applicable law.
Patients should not return or mail medications, used products, opened products, sharps, needles, syringes, biohazard materials, or temperature-sensitive items unless specifically instructed in writing.
Shipping, Pickup, and Storage
Once a product, medication, supplement, lab kit, or wellness item has been prepared, dispensed, shipped, picked up, or released to the patient, it cannot be returned, exchanged, or refunded.
Sublimeu Health & Wellness is not responsible for delays, damage, loss, theft, improper storage, missed delivery, incorrect delivery information provided by the patient, failure to retrieve products, or failure to follow storage instructions once the item has been transferred to the carrier, released to the patient, or made available for pickup.
Patients are responsible for providing accurate contact information, shipping information, and pickup availability.
Treatment Packages, Deposits, Memberships, and Prepaid Services
Treatment packages, prepaid services, memberships, deposits, promotional offers, and discounted services are non-refundable. Unused services may not be exchanged for cash.
Packages and prepaid services are intended for the original patient only and may not be transferred to another person unless approved in writing by Sublimeu Health & Wellness.
Any credit, transfer, extension, or exception is at the sole discretion of Sublimeu Health & Wellness and must be approved in writing.
Promotions and Discounts
Promotions, discounts, special pricing, and limited-time offers are not retroactive. Promotional pricing cannot be applied to prior purchases, prior visits, completed services, or previously paid packages.
Promotional services are subject to clinical eligibility and provider approval. Paying for or requesting a promotional service does not guarantee that the patient is medically eligible to receive that service.
Cancellations, Late Cancellations, and No-Shows
Appointment cancellation and no-show policies apply separately and may result in loss of deposit, appointment fee, prepaid credit, or service credit.
Missed appointments, late cancellations, failure to complete required intake forms, failure to complete labs, failure to attend follow-up appointments, or failure to provide required medical information may affect treatment eligibility and may result in additional fees.
Side Effects, Reactions, and Treatment Response
Side effects, medication intolerance, allergic reactions, lack of response, lack of desired outcome, change in treatment plan, or a patient’s decision to stop treatment do not guarantee a refund.
Patients should contact Sublimeu Health & Wellness promptly with any concerns so the provider can review the treatment plan and provide appropriate recommendations. Depending on the concern, the provider may recommend monitoring, dose adjustment, discontinuation, lab testing, referral, urgent care, or emergency evaluation.
Patient Responsibility
Patients are responsible for providing accurate and complete medical history, medication lists, allergies, surgical history, health updates, lab results, and any relevant outside records.
Patients are responsible for following treatment instructions, medication instructions, storage instructions, lab monitoring requirements, follow-up recommendations, and safety guidance.
Failure to provide accurate information, failure to follow medical advice, failure to complete recommended monitoring, or failure to attend follow-up appointments may affect treatment safety, outcomes, and eligibility for continued care.
Right to Refuse, Pause, or Discontinue Treatment
Sublimeu Health & Wellness reserves the right to refuse, pause, modify, or discontinue treatment if it is determined that a service, medication, product, or treatment plan is not medically appropriate, unsafe, contraindicated, outside the scope of the practice, or not in the best interest of the patient.
Refusal, pause, modification, or discontinuation of treatment does not guarantee a refund for services already provided, products already prepared, products already released, labs already ordered, or clinical time already completed.
Insurance, Self-Pay, and Reimbursement
Sublimeu Health & Wellness does not guarantee insurance coverage, reimbursement, payment, prior authorization approval, medication coverage, laboratory coverage, pharmacy coverage, or payment from any insurance company, employer, health plan, pharmacy benefit manager, or third-party payer.
Patients are responsible for understanding their insurance benefits, out-of-pocket costs, medication coverage, laboratory coverage, and financial responsibility.
If the patient chooses not to use insurance or if a service is not covered by insurance, the patient remains responsible for all self-pay fees.
Good Faith Estimate Notice
Uninsured and self-pay patients have the right to receive a Good Faith Estimate explaining the expected cost of scheduled healthcare services.
A Good Faith Estimate is only an estimate and is not a guarantee of final charges. Actual charges may differ if additional services, labs, treatments, prescriptions, follow-ups, or clinical needs arise.
Patients may request a Good Faith Estimate before scheduling or receiving services. Receiving a Good Faith Estimate does not require the patient to receive services from Sublimeu Health & Wellness.
Telehealth Services
For telehealth services, patients consent to receiving care through electronic communication platforms. Patients understand that telehealth may have limitations compared to in-person care.
The provider may recommend in-person evaluation, urgent care, emergency care, laboratory testing, or referral to another provider when clinically appropriate.
Patients are responsible for being located in a private and safe environment during telehealth visits and for providing their accurate physical location at the time of service.
Sublimeu Health & Wellness does not guarantee that every condition, concern, medication request, or treatment request can be managed by telehealth.
Website, Email, Text, and Portal Communication
Website messages, emails, text messages, portal messages, and online forms may not be reviewed immediately and should not be used for emergencies.
Patients should not use website forms, email, text message, or portal messaging for urgent medical concerns, psychiatric emergencies, medication reactions requiring immediate attention, suicidal thoughts, thoughts of harming others, chest pain, difficulty breathing, severe allergic reaction, or any emergency.
Privacy Notice
Sublimeu Health & Wellness is committed to protecting patient privacy and health information. Patients will be provided access to the practice’s Notice of Privacy Practices, which explains how protected health information may be used and disclosed, as well as patient rights regarding their health information.
This Returns, Refunds, and Service Policy does not replace the Notice of Privacy Practices.

Chargebacks and Payment Disputes
Patients are encouraged to contact Sublimeu Health & Wellness directly with any billing or service concerns.
Filing a payment dispute or chargeback for services rendered, appointments reserved, products prepared, products released, labs ordered, or treatments provided may result in account review, cancellation of future appointments, discontinuation of non-emergency services, and/or collection activity where permitted by law.
Collections and Outstanding Balances
Patients are responsible for all balances owed for services, appointments, products, labs, fees, or treatments provided. Outstanding balances may need to be resolved before additional non-emergency services are scheduled.
Sublimeu Health & Wellness reserves the right to use lawful collection methods for unpaid balances.
Policy Updates
Sublimeu Health & Wellness reserves the right to update, modify, or replace this policy at any time. The version posted on the website at the time of booking, purchase, payment, or service will apply.
