Sublimeu Health & Wellness does not guarantee specific results, symptom improvement, weight loss, aesthetic outcome, medication response, treatment success, diagnosis, prescription approval, lab outcome, or timeline of improvement.
Results may depend on many factors, including but not limited to age, medical history, genetics, metabolism, hormones, nutrition, activity level, medication adherence, lifestyle, sleep, stress, underlying medical conditions, laboratory findings, follow-up compliance, and patient adherence to the recommended treatment plan.

Appointments, Cancellations, and No-Shows

Late cancellations, missed appointments, failure to complete required forms, failure to complete labs, or failure to provide required medical information may result in cancellation fees, loss of deposit, loss of prepaid credit, or inability to proceed with care.

Payments and Financial Responsibility

Fees are charged for professional time, clinical review, assessment, documentation, medical decision-making, treatment planning, and appointment time reserved, regardless of whether a prescription, treatment, lab order, referral, clearance, diagnosis, or specific outcome is provided.
Returns, Refunds, and Service Policy
All sales are final and non-refundable except where otherwise required by applicable law.
By booking an appointment, purchasing a service, submitting payment, paying a deposit, purchasing a product, or participating in care, the patient agrees to the Sublimeu Health & Wellness Returns, Refunds, and Service Policy.
Patients should review the Returns, Refunds, and Service Policy before completing any payment, booking, or purchase.
Insurance and Reimbursement
Sublimeu Health & Wellness does not guarantee insurance coverage, reimbursement, payment, prior authorization approval, medication coverage, laboratory coverage, pharmacy coverage, or payment from any insurance company, employer, health plan, pharmacy benefit manager, or third-party payer.
Patients are responsible for understanding their own insurance benefits, out-of-pocket costs, medication coverage, laboratory coverage, and financial responsibility.
If a patient chooses not to use insurance or if a service is not covered by insurance, the patient remains responsible for all self-pay fees.
Good Faith Estimate Notice
Uninsured and self-pay patients have the right to receive a Good Faith Estimate explaining the expected cost of scheduled healthcare services.
A Good Faith Estimate is only an estimate and is not a guarantee of final charges. Actual charges may differ if additional services, labs, treatments, prescriptions, follow-ups, or clinical needs arise.
Patients may request a Good Faith Estimate before scheduling or receiving services.
Laboratory Services
Laboratory testing may be recommended or required based on the patient’s medical history, symptoms, medications, treatment plan, or safety needs.
Laboratory fees may be separate from consultation fees unless specifically stated in writing.
Lab results may affect treatment eligibility, prescription decisions, medication dosing, follow-up frequency, and clinical recommendations.
Sublimeu Health & Wellness is not responsible for delays, canceled tests, specimen issues, laboratory processing times, third-party lab billing, insurance denial, or laboratory policies outside of the practice’s control.
Medications, Prescriptions, and Refills
Prescriptions are issued only when clinically appropriate and safe.
Prescription requests, refill requests, or medication changes may require an appointment, updated medical history, lab monitoring, review of outside records, or additional clinical assessment.

Patients are responsible for taking medications as prescribed and notifying the provider of side effects, allergic reactions, medication changes, pregnancy, new diagnoses, new labs, hospitalizations, or changes in health status.
Compounded Medications, Supplements, and Medical Products
Compounded medications, supplements, injections, hormones, peptides, skincare products, wellness products, and customized medical products may have specific storage, handling, expiration, and use instructions.
Patients are responsible for following all medication, product, storage, injection, and safety instructions.
Due to health, safety, hygiene, storage, and regulatory reasons, these items may not be returnable or refundable once purchased, prescribed, prepared, dispensed, shipped, opened, picked up, or released to the patient.
Telehealth Services
Sublimeu Health & Wellness may offer telehealth services when clinically appropriate.
By participating in telehealth, the patient consents to receiving care through electronic communication platforms and understands that telehealth may have limitations compared to in-person care.
The provider may recommend in-person evaluation, urgent care, emergency care, laboratory testing, or referral to another provider when clinically appropriate.
Patients are responsible for being located in a private and safe environment during telehealth visits and for providing their accurate physical location at the time of service.
Sublimeu Health & Wellness does not guarantee that every condition, concern, medication request, or treatment request can be managed through telehealth.
